Take the Right ID

YOU MUST PRESENT ONE OF THESE IN ORDER TO VOTE:

  • A valid Ohio Driver’s License or State of Ohio Photo Identification Card (These IDs may be used even if the address is not current and does not match the address in the signature poll book); or

  • A Photo ID from the federal government showing your current address; or

  • One of the following that shows your name and current address:

  • Paycheck

  • Copy of a Current Utility Bill

  • (Issued within a year of Election Day)
  • Government check
  • Bank Statement
  • Government document

  • (Note: You may not use the notice you
    received from the Board of Elections.
    You may use documents like
    government letters, tax bills, license
    renewal notices, court papers, and
    public university or college transcripts
    or grade reports.)
  • Military ID

  • (Note: Most military IDs do not show
    address and will therefore not be valid.)






IF YOU ARE A REGISTERED VOTER, BUT DON’T HAVE ANY FORM OF ACCEPTABLE ID WHEN YOU GO TO THE POLLS, YOU STILL HAVE THE RIGHT TO CAST A PROVISIONAL BALLOT.

If you have to cast a provisional ballot, just check with the poll judges to make sure that you are in the correct precinct.  Then do ONE of the following things to make sure that the provisional ballot is counted.
  1. If you know your Social Security Number

  2. Fill out the Provisional Ballot Affirmation form with as much information as you can at the polling place and sign it.  MAKE SURE that you provide that last 4 digits of your Social Security number on the Affirmation.  If you do this then you do not have to go to the Board of Elections office to show identification after the Election; OR

  3. If you have a Social Security Number but can’t remember it

  4. Fill out the Provisional Ballot Affirmation form with as much information as you can at the polling place and sign it. Then, within 10 days of the Election, go to your County’s Board of Elections and provide either valid Voter ID or the last 4 digits of your social security number; OR

  5. If you do not have a Social Security Number

  6. Fill out the Provisional Ballot Affirmation form with as much information as you can at the polling place and sign it. Then, while you are still at the polling place, you should also fill out and sign a special affirmation form called a 10 T Form. If you refuse to fill out the 10 T Form at the polling place, you can still go to your County’s Board of Elections within 10 days of the Election, and provide either valid Voter ID or sign the 10 T Form at the Board’s office.
